Initial Assessment

Assessment is a crucial component of the NDIS Psychosocial recovery coaching process. It serves as the first step in understanding the individual's needs, strengths, and goals, and is a collaborative effort between the coach and the individual. By gathering information from multiple sources, the assessment process enables the coach to gain a holistic understanding of the individual's physical, emotional, social, and practical needs.

The coach begins by establishing rapport with the individual, providing a safe and non-judgmental environment for them to express themselves. This is followed by a series of questions designed to gather information about the individual's current situation, including their living situation, employment or education status, and social support network. The coach will also explore the individual's mental health history, including any previous diagnoses, treatments, and hospitalizations, as well as any current or previous substance use.

In addition to gathering information from the individual, the coach may seek input from family members, friends, or other healthcare professionals involved in the individual's care. This information can help the coach gain a broader perspective on the individual's situation and support needs.

The coach will conduct a functional assessment to understand the individual's practical needs, including their ability to perform daily living activities and their mobility. Throughout the assessment process, the coach will work collaboratively with the individual to identify their strengths and challenges. This information is critical in developing a recovery plan that is tailored to the individual's specific needs and circumstances.

Once the assessment is complete, the coach will use the information gathered to develop a recovery plan that outlines the individual's goals, the steps needed to achieve these goals, and the support required to do so. This recovery plan is a collaborative document that is reviewed and adjusted regularly based on the individual's progress.
